Stewarts Care is a long-established voluntary organisation providing comprehensive community-based services to people with intellectual disabilities.

We support people across a wide range of services within residential settings, day services, supported independent living, job advocacy and vocational training.

Purpose of the Post:

Following successful commencement our Community Liaison Nurse Team we are now recruiting to expand the team. The Community Liaison Nurse (Intellectual Disability – ID) will be a valued member of our Stewarts Community Liaison Nurse Team, supporting the health and wellbeing of residents in community services and ensuring clinical practice is underpinned by NMBI requirements and Stewarts Care Policies, Procedures, Protocols and Guidelines.

Candidates must meet the following criteria relevant to the role:

  • Currently registered with Nursing and Midwifery Board of Ireland in the RNID Division.

  • Applications from other nurses registered with Nursing and Midwifery Board of Ireland who have a minimum of 1 years’ experience in the area of Intellectual Disability will also be considered.

  • Candidates will possess a high level of communication skills, work well within a team and be able to provide high quality focused care.

  • Must have strong command of English language (both verbally and written) and be eligible to work in Ireland.

  • Candidates should be proficient in I.T.

  • Full driver’s license and access to your own car.

Salary and qualifications are in accordance with Department of Health consolidated pay scales. Salary Scale is Staff Nurse, Mental Health, €34,664-€51,952 inc. LSI per annum.

Job Purpose

Key Duties and Responsibilities

Professional Responsibilities

The post holder will:

  • Manage assigned caseload of community residents execute time management with line manager across assigned designated centres.

  • Develop and complete health assessments and annual medical reviews in line with the regulations of the Health Act in addressing existing and emerging health needs for all residents on caseload.

  • Provide nursing supports and advice to Residents on caseloads and their carers with regard to healthcare.

Annual Medical Review & Health Promotion

  • Support the resident and carer to attend and participate in their AMR.

  • Ensure follow up on recommendations from the AMR.

  • Support referrals and follow up to GP, Clinicians, nurse specialist services etc.

Healthcare Supports

  • Support the resident to prepare for and attend OPD to maximise opportunity and participation.

  • Medication management- support RSAMM trained staff competencies. Medication reviews and referrals and development of person-centred management plans.

  • Develop evidence-based healthcare plans using the nursing process for identified conditions from AMR/Hospital to ensure individual person-centred clear plan of care for Residents and their carers.

To include:

  • Information on the condition and how it relates to the person e.g. diagnosis, history, symptoms experiences, services, audit of records

  • How this condition specifically affects the individual

  • Self-care and support needs to manage the condition

  • How will the condition/symptoms be managed and key supports needed.

  • Signs and symptoms of deterioration and how this will be managed.

  • Robust review and evaluation to ensure person centred needs.

  • Provide information and education for residents and direct carers on developed healthcare plans.

  • Provide rotational GP appointment support for the assessment, referral, and appointment attendance and follow up, care plan and implementation to support acute healthcare needs and handover to case manager nurse.

  • Monitor and review of health-related recording forms eg. BGL, Bowel, weight, observation and support GP/Clinical referrals arising from recorded data as required.

  • Support clinical nursing colleagues in healthcare planning eg. Phlebotomy, Immunisation, diagnostic.

  • Develop and review the HSE healthcare passport to reflect current needs and supports.

  • Support Resident with person centred needs in identifying requirements for and attending screening programmes and diagnostics e.g. National screening programmes, phlebotomy, bone density etc.

  • Communicate effectively with Resident, carer and acute medical nursing team when inpatient for planned or emergency hospital admission to ensure effective communication, recording of progress/presentation and support safe discharge planning.

Communication and multi-disciplinary supports

  • Engage with PIC regarding residents’ healthcare needs and in particular declining/evolving needs and supports.

  • Support residents with assisted decision making with regard to healthcare by optimising access to information and applying reasonable accommodations.

  • Engage with outreach and community health services in support of individual residents needs eg. Epilepsy outreach, community palliative care, public health, Dental, Optical etc.

  • Attend and contribute at required MDT and annual review for Residents on Caseload with regard to healthcare needs.

Nursing Practice

  • Record nursing practice on eclipse system in a timely manner in accordance with NMBI requirements.

  • Engage and refer to clinical team where required.

  • Involvement in health promotion and chronic disease prevention fora and education programmes.

  • Support the development and review of SOP’s required for nursing practice.

  • Ensure personal time management and consultation with line manager with regard to assigned caseload.

  • Ensure nursing competency and scope of practice in continued CPD in the area of chronic disease management, health promotion and specific needs of persons on the caseload.

  • Deliver and report on KPI’s and updates on caseload to the line manager for compliance tracking. Engaging in team meetings with line manager in relation to team and service planning

  • Support audit and reports with regard to HIQA regulations on healthcare.

  • Provide preceptorship and mentorship for undergraduate students.

  • Evidence based practice in line with national and international best practice and policy e.g. National wound care guidelines,

  • Manage upkeep and levels of equipment and supplies required in the role.

Candidates must demonstrate sufficient competency and professional knowledge to carry out the duties and responsibilities of the role.

Education and Skills

Essential Criteria:

  • Excellent communication skills both verbal and non-verbal.

  • Minimum one years’ experience in ID.

  • The aptitude to provide a person-centred approach to service user programmes.

  • The ability to develop implement and monitor such individual programmes.

  • Full driving license and access to own car.

  • Demonstrates knowledge of the services provided by the organisation.

  • Knowledge of the organisational structure and functions.

  • Awareness of the organisation’s Strategic Plan 2016-2019.

  • Understanding the importance of the MDT in the provision of care and the ability to develop and maintain relationships with all stakeholders.

  • Proficient in Microsoft suite and strong ICT skills relevant to the role.

  • Ability to work on own initiative.

Remuneration

Remuneration is in accordance with the Department of Health consolidated pay scales. 

Current Salary Scale: Staff Nurse, Mental Health, 

Salary Scale: €34,664-€51,952 inc. LSI per annum.

Salaries and increments are paid in accordance with DOH salary scales

Annual leave

To be agreed at time of offer.

Working week

The hours allocated to this post are 37.5 hours per week exclusive of lunch breaks. The allocation of these hours will be at the discretion of the Director of Nursing and in accordance with the needs of the service.

Probation

All employees will be subject to a probationary period of 6 months, with an extension if required to 9 months. This probationary period applies to all employees irrespective of their role or type of role.

Garda Vetting

Stewarts will carry out Garda vetting on all new employees. An employee will not take up employment with the organisation until the Garda Vetting process has been completed and the organisation is satisfied that such an appointment does not pose a risk to the service users and employees.
