Overview of the Post

The family Support Worker will work as a member of the interdisciplinary team in collaboration with MDT colleagues providing services to children from 0-18 years and families who are accessing the Children's Disability Network Team.

The family support worker will provide support to the child and family in caring for their child with complex needs.  This will involve supporting the family to access local community-based supports and support the family through a range of advocacy and communication supports when liaising with outside agencies. 

They will help to facilitate the parents to sustain the safe care of their child in the home setting and support the child and family with the implementation of various aspects of their therapy programmes in line with the Child’s Individual Support Plan. 

The Family Support Worker will work through a family centered model of practice in line with the principles of Progressing Disabilities Servies.

The role involves close work with all members of the Interdisciplinary team and close work with the Senior Social Worker.

Professional clinical supervision will be provided.

Continuous Professional Development opportunities will be provided.

Essential

  • Evidence of having completed Fetac Level 5 or other relevant Health/Social care qualification.

  • A minimum of 2 years' post-qualification experience of working within the disability Sector.

  • A broad knowledge of Progressing Disabilities and the role of a Family Support Worker.

  • Access to own Transport.

  • Experience of working with families & children

  • Experience of working in Disability

  • Experience of Groups work

Job Purpose

To work as a member of the interdisciplinary team providing services to children and families within the region. The post holder will assist in the provision of family centred services to children accessing intervention services as appropriate.
Services are provided to children aged 0 - 18 years who present with complex needs arising from a disability.

Scope of the Role

The role of family support worker will be to facilitate the child with a disability and their family, in all aspects of daily living including assisting with accessing services in the community, supporting families with liaison with external agencies and advocating for their needs, and support with implementing aspects of therapy programmes as advised by the interdisciplinary team through a family centred approach.

The role of the family support worker is tailored to the requirements of the individual service user and their family and as such will vary from person to person.  The Family Support worker will link closely with the Professionally Qualified Social Worker and the wider Multidisciplinary team in supporting the overall care plan for the child and family.

Key Duties and Responsibilities

Key Responsibilities

  • To observe and maintain a code of confidentiality and discretion in all dealings with service users, their families and with staff.

  • To always preserve the professional nature of the relationship with service users.

  • To promote and safeguard the well-being and interests of the service user and their family at all times.

  • To undertake only such duties delegated and/or allocated to you by the Children’s Disability Network Manager or person nominated by them.

  • To perform all duties and tasks in line with Stewarts Care guidelines and policies, and training.

  • To immediately report any accidents or incidents/dangerous occurrences immediately to the CDNM.

  • Facilitating service user with involvement in their local communities.

  • Supporting Service user with outings and accessing leisure activities within their own neighbourhood or within the wider community in line with the goals identified in their IFSP.

  • Working with members of the Multidisciplinary Team in carrying out Individual Family Support Planning meeting with Children and young people and their families.

  • Supporting the child and family to achieve goals as identified in their Individual Family Support Plan this may be through direct intervention, supporting at groups interventions or through a range of advocacy and supportive roles. 

  • Supporting family with welfare rights, entitlements and liaison with external agencies in the context of supporting the child/family with their IFSP goals.

  • To assist with the delivery of therapeutic and targeted intervention groups with other members of the Multidisciplinary team.

  • Carry out all administrative duties as directed by the CDNM or person nominated by them relating to the role.

  • Keeping up to date records of all interventions with children and families.

  • Any other duties which may be assigned from time to time.

  • Support and enable parents to sustain the safe care of their children in the home setting.

Qualifications & Experience

Candidates must meet the following criteria relevant to the role

  • Evidence of having completed Fetac Level 5 or other relevant Health/Social care qualification.

  • A broad knowledge of Progressing Disabilities and the role of a Family Support Worker.

  • An understanding of disability issues.

  • An awareness of boundaries and confidentiality in relation to the role of the Family Support Worker.

  • Access to own transport

  • An understanding of the Social Model of disability and person-centred approach

  • Training in or experience working with children and families in the disability Sector is desirable.

Remuneration

Remuneration is per the Department of Health consolidated pay scales 

Current Salary Scale: Family Support Worker

€32,629 to €41,688 per annum.

Salaries and increments are paid as per DOH scales

Working week

The hours allocated to this post are 39 hour standard working day, exclusive of lunch breaks.

The allocation of these hours will be at the discretion of the CDNM and in accordance with the needs of the service.

Probation

All employees will be subject to a probationary period of 6 months, with an extension if required to 9 months. This probationary period applies to all employees irrespective of their role or type of role.

Garda Vetting

Stewarts will carry out Garda vetting on all new employees. An employee will not take up employment with the organisation until the Garda Vetting process has been completed and the organisation is satisfied that such an appointment does not pose a risk to the Service Users and employees.
